Continuity of patient management between GPs and specialists is key to good healthcare. A threat to this ideal is when drugs are prescribed and administered solely under specialist care or as part of clinical trials, without being recorded in GPs' computer systems. Common examples include isotretinoin and cytotoxic drugs, and the consequences could be disastrous, in terms of unsuspected interactions or unwanted effects.
