The risks of accidental dosing errors with methotrexate have been publicised for more than a decade.¹ Despite efforts to reduce the risk, errors continue to be reported, and the Spanish medicines regulator (Agencia Española de Medicamentos y Productos Sanitarios) has asked the European Medicines Agency to review error reports, to find out why they are still happening and to decide whether further action should be taken to prevent them.²
