Those with an ultra cautious view of pharmacotherapy would say that every new prescription is a one-off trial with the intent of altering a process in the body for the better. However, modern clinical practice involves shared decision-making that only initiates therapy following an honest conversation with our patients, where goals and follow-up have been defined, leading to a concordant therapeutic plan. Such an approach is especially important for the management of long-term conditions where outcomes can be less patient-centred and where treatment options require frequent monitoring.
