RT Journal Article SR Electronic T1 Clarification: netilmicin in endocarditis (6 March, p19) JF Drug and Therapeutics Bulletin JO Drug Ther Bull FD BMJ Publishing Group Ltd SP 40 OP 40 DO 10.1136/dtb.27.10.40 VO 27 IS 10 YR 1989 UL http://dtb.bmj.com/content/27/10/40.abstract AB We suggested that netilmicin might be preferable to gentamicin in endocarditis caused by faecal streptococci to reduce unwanted effects from prolonged treatment. “Faecal streptococci” comprise two species, Strep faecalis, and the rare Strep faecium which is innately resistant to netilmicin as we noted in 1982. Both species are increasingly showing high-level gentamicin resistance in America. Laboratory tests are essential to determine optimal treatment, and netilmicin should not be universally substituted for gentamicin in endocarditis caused by unspeciated “faecal streptococci”.
