In 2019, we highlighted the important role that community pharmacies can play in supporting patients with their medicines on discharge from hospital.1 Recent publications have provided a helpful reminder that there is a high rate of medication error or discrepancy after discharge,2 that older people experience considerable difficulties managing their post-discharge medicines,3 and that community pharmacy post-discharge care schemes are considered to offer benefits to patients and the wider healthcare system.4 Earlier this year, a funded community pharmacy discharge medicines service (DMS) was introduced in England, allowing hospital staff to refer patients who would benefit from extra guidance around their medicines regimen for support from a community pharmacy.5 For this …
